Office Hours are 8 a.m. to 5 p.m., Monday - Friday, and 8 a.m. to 12 noon, Saturday.

During your first visit, you will be asked to complete registration forms. Please arrive 15 minutes early to allow enough time to fill out these forms.

966-0210 or Contact the Family Medicine Center via E-mail as soon as possible to reschedule or cancel. E-mail and Voice messages are checked hourly.

Payment
The Family Medicine Center requests payment on the day of service. You will receive an itemized statement at the conclusion of your visit at which time you may pay the receptionist. In special cases, the Center will allow you to make payment arrangements with our financial counselor.
Each time you visit the center, a claim will be filed with your insurance company. All deductibles and co-payments are due on the day of service. The Center is a "participating" Medicare Provider and accepts Medicaid as well. Please bring your insurance or managed care card with you at each visit.
